Blood Substitutes

Blood substitutes are solutions designed for use in patients who need blood transfusions, but for whom whole blood is not available, or is not safe. This interest has intensified in the wake of the AIDS and hepatitis C epidemics. Blood Substitutes describes the rationale, current approaches, clinical efficacy, and design issues for all blood substitutes now in clinical trials. The many summary diagrams and tables help make the book accessible to readers such as surgeons and blood bankers, who have less technical expertise than the biochemists and hematologists who are designing and testing blood substitutes.

Intensive Care Medicine in 10 Years

Although its roots date back to the early decades of the 20th century, critical care medicine did not emerge as a specialty in medicine until the 1970’s. Over the last 30 years or so, the field of critical care medicine has grown tremendously and there is now a solid body of scientific information that forms the foundation for the practice of critical care medicine. But, now as physicians and hospitals around the world focus to an ever greater extend on providing high quality care, the practice of critical care medicine will continue to grow in visibility and importance among clinicians and the general public as well.
This book seeks to identify the trends in critical care medicine that will form the basis for practice over the next ten years. Predicting the future is always risky. Nevertheless, the ideas articulated in this book are likely to serve as a road map for intensivists, hospital administrators and governmental leaders interested in healthcare, as they seek to improve the quality and efficiency of hospital-based services.
Written for: Critical care physicians, physicians-in-training, physicians who manage critically ill patients (trauma surgeons, cardiac surgeons, pulmonary medicine physicians), hospital administrators.

Perineal and Anal Sphincter Trauma: Diagnosis and Clinical Management

There has been an increase in litigation due to a failure of recognition of anal sphincter tears and inappropriate management. This aims to clarify and graphically demonstrate perineal anatomy and explain the pathophysiology of anal incontinence as well as applied pharmacology. It also institutes the new recommended classification of perineal tears and describes the various anal sphincter repair techniques. The emphasis is placed on the correct post-operative management, management of a subsequent pregnancy following previous anal sphincter injury and prevention of anal sphincter tears as an important issue that has not been given due consideration previously. The authors also focus on medical/legal implications in order to introduce safe medical practice.
The book will mostly be of interest to trainees and consultants, colorectal surgeons, physiotherapists, midwives, continence advisors, labour wards and lawyers.

Anxiety Management in Adult Day Surgery: A Nursing Perspective

The book is the first of its kind to specifically outline the psycho-educational nursing interventions required by the anxious, adult patient undergoing elective, ambulatory surgery. Anxiety management is a considerable issue for the majority of surgical patients and has been recognised as such for many decades. However, no formal nursing intervention currently exists to support patients during this acute phase.
This book is one of the first to provide strong evidence for the way in which patients can be assisted in the management of their anxiety. Moreover, it provides future direction for surgical nursing intervention in this new era of minimal invasive surgery where patients undergoing elective procedures increasingly require less physical nursing intervention and spend very little time within the acute hospital setting.

Sodium Channels, Pain, and Analgesia (Progress in Inflammation Research)

Sodium channels confer excitability on neurons in nociceptive pathways and exhibit neuronal tissue specific and injury regulated expression. This volume provides recent insights into the control of expression, functioning and membrane trafficking of nervous system sodium channels and reviews why sodium channel sub-types are potentially important drug targets in the treatment of pain. The roles of sodium channels in dental and visceral pain are also addressed. The emerging role of sodium channel Nav1.3 in neuropathic states is another important theme. Authors from the pharmaceutical industry discuss pharmacological approaches to the drug targeting of sodium channels, and in particular Nav1.8, exclusively expressed in nociceptive neurons. The final chapter highlights the functional diversity of sodium channels in part provided by post-transcriptional processing and the insights into sodium channel function that are being provided by tissue specific and inducible gene knock-out technology.

Emergency Medicine (Vademecum)

The idea when we started was to collect the core Emergency Medicine
information and present it in an abbreviated, succinct manner, useful to housestaff and medical students. As we progressed it became obvious that the very breadth of the specialty prevented any one person from accomplishing this task. It also became obvious that our specialty had advanced past the point where succinctness was possible. We peeled, boiled and pared, and came up with this.

Great Feuds in Medicine :Ten of the Liveliest Disputes Ever

In 1761 German physician Leopold Auenbrugger remarked, "It has always been the fate of those who have illustrated or improved the arts and sciences by their
discoveries to be beset by envy, malice, hatred, destruction and calumny." Following Great Feuds in Science, Hellman (Beyond Your Senses) now documents 10 dramatic medical disputes. British anatomist William Harvey's 1628 discovery of blood circulation challenged anatomical theory and caused his ostracism by the scientific community. In the late 18th-century, electrical disputes raged between Galvani whose "animal electricity" theory, to modern sensibilities, borders on the occult, though it garnered immediate support and prefigured current studies of electricity and paralysis and Volta, who worked to disprove Galvani. Claude Bernard's experiments on animals, in his studies of the nervous system, caused outrage among antivivisectionists and led to his being disowned by his family. Hungarian obstetrician Ignaz Semmelweis was committed to a mental hospital after the Viennese medical establishment rejected his hypothesis that unsanitary conditions in his workplace (doctors went from an autopsy to a birth without washing their hands) caused the high infection and death rate among patients. Other chapters address "Freud bashing," Sabin vs. Salk (polio vaccine) and Gallo vs. Montagnier (AIDS). Hellman eschews comprehensiveness for pith and entertainment, neglecting no unusual "twist," "strange coincidence," "cloud of suspicion" or "lucky break" to heighten the drama of these medical milestones. (Mar. 9)Forecast: Hellman's well-received books and articles (in the New York Times, Omni, Reader's Digest, etc.), and the success of popular science writing ensure his continuing appeal to serious science buffs, history fans, and casual readers alike.

Fluid Volume Monitoring with Glucose Dilution

Decision making regarding fluid volume loading, fluid restriction, or administration of vasoactive drugs may vary among physicians, depending solely upon their clinical experience in the absence of evidence-based measurement. The initial distribution volume of glucose (IDVG) is believed to be clinically relevant as a marker of cardiovascular and fluid management in critically ill patients without a significant modification of glucose metabolism. This book covers all aspects of IDVG measurement, including the basic concept, its relationship with other fluid volumes, and the clinical application of this technique in the intensive care unit based on both the author's studies and their clinical experience with more than 4000 IDVG determinations. When the concept of IDVG is properly understood and its measurement is then performed routinely, daily fluid and cardiovascular management in critically ill patients can be improved saded on evidence-based measurement
A clinically important misinterpretation of cardiac preload or circulating blood volume could cause serious consequences in critically ill patients.
However, many physicians do not possess a proper knowledge of fluid volume assessment. They are generally not interested in fluid volume assessment, but rather are interested in the underlying pathology and/or cardiac function of their patients. Accordingly, decision making about fluid volume loading, fluid restriction, or administration of vasoactive drugs may vary among physicians, depending solely on their clinical experience in the absence of evidence-based measurement, leading to a significant increase in morbidity as well as length of hospital stay in the critically ill.

Handbook on Hyperbaric Medicine

Hyperbaric oxygen application has now become a useful technique for both diagnostic and therapeutic purposes in CNS, cardiovascular and respiratory diseases, as well as in soft-tissue and orthopaedic pathologies and haematologic disorders. With a specific didactic approach, supported by numerous illustrations and tables, this volume aims to present all aspects of oxygen application under pressure not only to resolve some clinical problems, but also to improve recovery or to modify a negative illness evolution. Both scientists and practitioners will find this work a useful and updated reference book.
It is now ten years since the first Handbook on Hyperbaric Medicine was published. During this time there have been many major advances: our understanding of the actions of hyperbaric oxygenation has been elucidated by several studies; clinical practice is becoming more scientific; various consensus-derived organisational and operational recommendations and guidelines are now widely accepted.
As for Europe, these positive developments are largely due to the continuous action of the European Committee for Hyperbaric Medicine (ECHM). One of the most successful initiatives was the start of a specific European research action sponsored by the EU Co-operation in Science and Technology (COST) programme. The specific COST Action for hyperbaric medicine, COST B14, has been completed and has provided the impetus for the publication of this new Handbook.
This book is a reference document for researchers and clinicians, and is also of excellent use for both teachers and students.
Written for: Hyperbaric and diving physicians; researchers in high pressure biology; intensivists and researchers in oxygen biology; physicians connected with one of the major indications of HBO: general surgery, orthopedics and traumatology, plastic surgery, oncology radiotherapy, diabetology, ENT, etc.; medical libraries, industry (diving companies, hyperbaric chamber and equipment manufacturers), regulatory organisations (public health agencies, Normalisation
Institute).

The ECG Made Easy, 6th edition

This highly respected book is a simple, readable guide to the accurate identification and interpretation of normal and abnormal electrocardiogram (ECG) patterns for medical students, nurses and junior doctors. The emphasis throughout is on the straightforward practical application of the ECG. It will prove useful to all medical and health care staff who require clear, basic knowledge about the ECG.
The ECG Made Easy has for some time been the pocket reference for junior doctors... good starter, but you may need some help. One of the most popular introductory texts on this subject. All the basic concepts for understanding this topic are included, though there could be more emphasis on possible areas of confusion; such as the different between leads and wires . This book also suffers from only being pocket sized and having pocket sized tracings, and on pages that depict limb and chest leads the layout could be improved to show that they are separate groups. For a newcomer to this subject you may not find it all that easy, as the explanations may need some practical demonstrations from an accomplished teacher with topics such as the cardiac axis. However this book does avoid the jargon and physiology that can be off putting in most other introductory texts. If this book leaves you with further questions, you would probably also need and indeed benefit from the author s two other titles The ECG in Practice and 100 ECG Problems . This would enable you to improve your skills of interpretation, and the three titles bought over time would provide an adequate inexpensive reference shelf for most health care workers.

150 ECG Problems

A practical resource for developing ECG interpretation skills. 150 12-lead ECG strips and their associated case histories challenge readers to formulate diagnoses for a wide variety of cardiac abnormalities. Detailed an-swers and explanations allow readers to see whether they reached the correct conclusions. And, page referen-ces to Dr. Hampton's other best-selling booksECG Made Easy and The ECG in Practicemake it easy to find more information on the relevant principles of ECG interpretation.
"For anyone who feels they are competent at reading twelve lead ECGs this would be good for sharpening your skills of diagnosis, or even as a refresher. I think this is the best read and best buy of John Hampton s three books on the ECG, the others being The ECG Made Easy and The ECG in Practice as the recordings are presented in random order and the readers interest should be maintained throughout. The tracings are all graded for difficult; medical student, house officer, paramedic or nurse, and MRCP standard. Each twelve lead ECG in this book has a case history, and the answers are brief but to the point; listing ECG features, clinical interpretation, and importantly What to do . With a minimal amount of text and plenty of recordings this is good value if you have grasped the basics as in The ECG Made Easy . This book will act as a handy pocket reference with its comprehensive index as all the recordings are cross referenced with The ECG Made Easy and The ECG in Practice . An enjoyable and challenging book for most health care workers.

Mechanical Ventilation, An Issue of Critical Care Clinics

It is not only our goal in the issue to review the basic aspects of mechanical ventilation, such as modes of mechanical ventilation, ventilatory monitoring, and weaning from ventilation, but we wish to also develop discussion on some of the common controversies such as alveolar recruitment and stabilization. This issue also addresses some of the cutting edge concepts of mechanical ventilation such as therapist-driven protocols to standardize our care within specific disease groups.
More and more, we use the education and expertise of respiratory therapists to provide a comprehensive team approach in patient management.
Introduction of computer-based closed loop ventilation will also surely change the face of critical care practice as we allow technology to aid us in the 24/7 environment of the modern ICU.
We have also started to address three major complex patient disease groups in this issue:massive chest trauma, the complex environment of the neurologic ICU, and the fast-track world of the cardiovascular patient. Aspects of each of these may have implications in the care of other less specialty- based patients. No issue of a monograph on mechanical ventilation
can be complete without focusing on a major question in our society, that of end of life. We are hopeful that this issue will be useful and spur broad-based discussion on how to wean mechanical life support during that highly emotionally charged period.

Mechanical Ventilation

Mechanical ventilation and weaning is one of the most common procedures carried out in critically ill patients. Appropriate management of these patients is of paramount importance to improve the outcome in terms of both morbidity and mortality. This book offers the physiological and clinical basis required to improve the care delivered to patients undergoing mechanical ventilation.
In recent years there have been major advances in our understanding of how to ventilate patients, when to initiate and discontinue ventilation, and importantly, the side effects of mechanical ventilation. This book represents a state-of-the-art review by the leading experts in this field and covers a number of important topics including epidemiology, underlying physiological concepts, and approaches to monitoring. The pros and cons of various modes of ventilation are reviewed, as are novel forms of ventilation that may play a role in the future management of patients with respiratory failure. The importance of patient-ventilator synchrony and ventilator-induced lung injury are reviewed, with a focus on recent clinical trials and the challenges of implementing the results into clinical practice.

Turning up the Heat on Pain: TRPV1 Receptors in Pain and Inflammation (Progress in Inflammation Research)

Vanilloids mediate their effects by selective agonism of an ion channel, the vanilloid receptor 1 (TRPV1). TRPV1 is a ligand-gated, non-selective, cation channel preferentially expressed in small-diameter, primary afferent neurons, including nociceptive sensory nerves. In addition to be activated by capsaicin and related vanilloids, TRPV1 responds to heat and extracellular acidification, and will integrate simultaneous exposures to these stimuli. The aim of this book is to summarize recent insights in the role of TRPV1 in pain and inflammation, and to discuss how modulation of this receptor may lead to important advances in analgesic drug development. Insights into the prospects for the therapeutic potential of TRPV1 activation or inhibition, particularly in various painful or inflammatory conditions, are provided. The volume is written for basic scientists and clinicians interested in the mechanisms of pain, inflammation and the treatment of these conditions.

Emergencies and Complications in Gastroenterology

Acute conditions in gastroenterology are typically connected with high morbidity and mortality; an optimal therapy of these conditions demands a close cooperation between several disciplines, particularly gastroenterology and surgery. However, systematic data for a generalized approach to determine the optimal diagnostic procedures and therapy have so far not been available. The publication on hand tries to fill this gap by providing gastroenterologists and surgeons with recommendations for a rational multidisciplinary approach.
Acute states are characterized by multifactorial etiological changes as well as polymorbidity, which have an adverse influence on diagnostic accuracy and effect of therapy. In this situation, endoscopic examination which enables a simultaneous therapeutical solution is of fundamental importance. However, endoscopy is an invasive method, and due to patients’ polymorbidity, endoscopic approaches are limited by their general clinical condition, particularly by cardiopulmonary compensation. In these cases, non-invasive diagnostic methods are called for, such as ultrasound, computer tomography (CT) or nuclear magnetic resonance imaging. Moreover, modifications of these methods, e.g. CT-enteroclysis or CT-colonography, provide very precise and immediate results that allow to adopt the optimal strategic method. These methods may in the future substitute endoscopic examinations.
This publication has been especially compiled for gastroenterologists, endoscopists, surgeons, intensivists and physicians involved in the management of acute cases to help them determine the optimal therapy for their patients.
